2022-23 Athletics Pre-Participation Paperwork

If you are interested in playing a varsity or club sport at Framingham State University during the 2022-2023 academic year, there are several required forms you need to fill out before you may begin participating and those forms are now available at the links below. Please be sure to read all instructions carefully.

All athletic paperwork will be completed electronically for the 2022-23 academic year. In order to be cleared for participation, all forms must be completed.

All paperwork is posted below and is split into two categories. 

  1. If you have never played a sport at Framingham State, please complete the list of paperwork under the New/Transfer Student-Athlete heading.
  2. If you have previously participated on a varsity or club team at Framingham State, please complete the paperwork under the Returning Student-Athlete heading. 

For those student-athletes wishing to play a fall sport (men's soccer, women's soccer, field hockey, football, men's cross country, women's cross country and women's volleyball), forms are due by August 1st.

For those student-athletes wishing to participate in a spring sport whose non-traditional season is in the fall (baseball, softball, women's lacrosse), forms should be submitted by August 1st.

For those student-athletes wishing to play a winter sport (men's basketball, women's basketball, ice hockey), forms should be submitted by September 15th. 

For those wishing to play a club sport in the fall, paperwork should be completed by September 1st.

Questions regarding the medical portion of the paperwork may be directed to head athletic trainer Laura Rusk at lrusk@framingham.edu.

Questions regarding all other paperwork should be directed to Assistant Athletic Director Kathy Lynch at klynch@framingham.edu.

COVID-19 INFORMATION: All student-athletes and members of the FSU community must comply with the University's COVID-19 protocols in order to participate in athletics. Please visit the FSU COVID website for the most up-to-date policies and information.

 

NEW/TRANSFER STUDENT-ATHLETES

  1. NCAA Registration – works best when completed using google chrome (club sport athletes do not need to complete this form and will not receive email from NCAA)
  2. Sports Information Questionnaire – works best when completed using google chrome
  3. Athletic Training Paperwork – Athletic Training Paperwork Instructions - please be sure to download forms before completing them
  4. Hazing Form - please return via email (klynch@framingham.edu)
  5. Sickle Cell Trait Status Documentation - NEW NCAA Requirement
  6. **Check your student-email account for an email from the NCAA with your compliance forms. Please allow a minimum of two weeks for processing after completing NCAA Registration.

 

RETURNING STUDENT-ATHLETES

  1. Sports Information Questionnaire – works best when completed using google chrome
  2. Athletic Training Paperwork – Athletic Training Paperwork Instructions - please be sure to download forms before completing them
  3. Hazing Form - please return via email (klynch@framingham.edu)
  4. **Check your student-email account for an email from the NCAA with your compliance forms.
